Skilled Nursing

Our nurses are highly trained professionals comprising registered nurses (RNs) and licensed practical nurses (LPNs) who coordinate and direct the patient’s care based on individual needs and deficits in the home setting. Our nurses are responsible for the independent management of a home health patient requiring the use of advanced assessment, teaching and decision-making skills. Home health nurses evaluate the patient for appropriateness of home health and develop the home care plan in conjunction with the physician. Nurses educate patients, families, caregivers and community providers to safely perform care that’s best for the patient.

Nursing provides follow up by evaluating effectiveness of the home health care plan, and monitoring patient/family’s response to the plan. Nurses are also responsible for ensuring that appropriate referrals to other services are made, interdisciplinary conferencing is taking place regularly, and appropriate documentation is completed.
