Adults with a serious mental health illness have a decreased life expectancy of up to 25 years compared to the general population, in large partly due to preventable physical health conditions. Our program utilizes a multidisciplinary approach deploying a psychiatric nurse, occupational therapist and physical therapist to improve the patient’s overall wellness with an emphasis on mental and physical health utilizing clinical care pathways. Our services improve health outcomes, improve the experience of care for the patient, improve quality of life and reduce health care costs.
